To provide domiciliary care including personal care and moving & handling. Also to review your medicines use and contact GP surgeries and other healthcare units regarding management of your appointments and prescriptions, to expedite social care assessments .

The pharmacist/ pharmacy technician will visit clients at their homes to ascertain the general environment and to understand how the clients take their medication, where they store their medication, to optimize medication taking and prescribing, detect any problems and adverse reactions and to communicate these to GP surgeries, social workers and health and social care institutions.

This service aims to provide a better integration between health and social care services leading to better outcomes for patients and service users.
These services are provided by pharmacists/ pharmacy technicians who trained as social care workers with continuity and constancy with your care by getting the same regular "social care pharmacists/pharmacy technicians", allowing that changes can be detected quicker and prompt actions will be undertaken.
